General Questions

For the most up to date and full list of office policies please refer to posted policies in office. Our in office policies list supersedes any policies post on this website. 

How do I book an appointment?

If you would like to become a patient of Dr. Rayhaan Dawood’s then please visit our office in person with a valid health card fill out a wait list form and review our office policies. You will then need to book an initial meet and greet appointment and bring all your current medications/past medical history to the appointment. 

How should I prepare for my appointment?

Please come to your appointment prepared by bringing all your current medications. All appointments are book for approximately 10 minutes. 

What services are not insured by OHIP?

OHIP covers the cost of your appointment as well as any medication prescriptions needed. The cost to do forms, make copies of your records, prescriptions/notes for medical devices, and referrals to alternative health care providers (physio, massage, orthotics) is not covered by OHIP. For a full list of uninsured services please visit the office. 

I have private medical coverage, can I still come see you?

Yes! We do accept patients with private plans such as UHIP as long as you are a “resident or citizen of Canada”. We unfortunately can not accept patient’s who are “non-residents” of Canada. Please be aware that you will need to pay our office directly for each visit if you do not have medical coverage through a provincial plan. Patient’s with Quebec Health Coverage must also pay our office directly for visits. You can submit the invoice to your private insurance for reimbursement. Please keep in mind that we do not guarantee how much your insurance will reimburse, that is between you and your insurance company.

I have private insurance, will they reimburse me for vaccines and forms?

Every insurance company is different. Some reimburse 100%, others at a much lower rate. The general advice we give is to check with your medical insurance first as we can not guarantee to what extent they will reimburse you. 

Policies:

Health Card Policy:

Your health card is like a driver’s license. You must bring your most current health card to every visit.

Appointment Policy and Missed Appointments:

Arrive before your appointment time. If you are late for your appointment, you may be accommodated but there is no guarantee. If you can not make your appointment for any reason you must let us know 24 hours or 1 business day in advance or you will billed a missed appointment fee. Same day cancellations may still be billed a fee.

Bring all medication bottles that you are currently taking to every appointment.

Disruptive Behavior Policy:

Your health is our priority. In the interest of delivering the best care possible we have adopted a a zero tolerance policy for disruptive behaviour. This includes: consistent failure to keep appointments/ threatening, demanding or abusive behavior/ deceptive behavior / medication abuse. Any of the above can and will likely result in immediate termination from the practice

Controlled Substances Policy:

As a policy the office does not prescribe narcotics, tranquilizers, or stimulants after only one visit and without all past medical records from previous providers; nor do we prescribe them for chronic reasons. We also do not provide prescriptions for medical cannabis. This is non-negotiable.

The office will only prescribe medications as indicated by the latest guidelines. We are very judicious when offering antibiotics and strong pain medications or tranquilizers/stimulants, especially when used in combination. You will be asked to sign an agreement if you are on one of the above medication classes. 

Medication Refill Policy:

It is your responsibility to monitor your prescriptions, if you are running low you must let your pharmacist know several weeks in advance so they can fax them to us for review. Also, refills by fax are only for chronic medications that have been approved by Dr. Rayhaan Dawood. Requests for old medications, antibiotics, pain killers, or other short term medications will likely not be renewed by fax. Also, excessive use of the prescription fax service or failure to attend an appointment when requested will result in a charge for renewals by fax.

Last minute requests will not be accommodated. Remember renewals by fax are a privilege not a right, and we reserve the right to say no or terminate this at any time.

No Recording Policy

Our office has a zero tolerance policy for recording of any kind. This includes video or audio recording in public spaces and examination rooms. Violation of this policy will result in immediate discharge from the practice. 

Walk In Clinic Policy:

We request that you do not use walk in clinics. For medical issues please call our office directly to book an appointment. For emergency care that can not wait we advise you to visit your local emergency room. Walk in clinics may be convenient at times, but they do not provide consistent care; and also result in negation to your family doctor’s office.  Use of walk in clinics may lead to discharge from the clinic.
